Relates to total amount being applied for, pending and in force with Sun Life Financial within the last 12 months of the date the most recent application was signed.

Definitions/Explanations:

Non-medicals can be replaced by tele-interview (new business only).
† Paramedicals can be replaced by tele-interview and vitals (new business only).
† If completed application does not include non-medical declarations, a paramedical will be required in place of vitals. * Refers to advisor-collected saliva specimens. If collection is done through a paramedical facility, obtain a urine/HIV.

  • Non-medical: Completion of all medical and risk selection questions on the application.
  • Mature age focus interview will be automatically completed by the examiner for all clients over age 75.
  • Proposed insureds age 16 or older who have lived in Canada for less than one year will require a paramedical and blood (with hepatitis B and C markers) as a minimum or, if more extensive, the regular requirements in the chart above.
  • A medical and saliva will always be ordered by the advisor. The inspection report, motor vehicle report and tele-interviews will always be ordered by Head Office personnel. All other requirements will be ordered by the advisor or Head Office personnel as indicated in the application.

Requirements for special plans and benefits:

Guaranteed insurability benefit (GIB): For children under age 10, ignore GIB in determining medical requirements. For ages 10 and over, add two GIB options to the face amount to determine underwriting requirements.
For Executive GIB, add the total of all options to the face amount to determine underwriting requirements.
Waiver of premiums on the life of the owner: For ages 55 and under, non-medical limits apply up to $2,500 annual premium. For higher ages and amounts, a paramedical is required.

Reinstatements:

Requirements include the appropriate Application for reinstatement. In addition, regular evidence for age and amount (as indicated above) must be submitted:

• when the reinstatement is more than 42 days from the date the policy went out of force and the amount is $200,001+, or • when the reinstatement is more than 90 days from the date the policy went out of force, regardless of the amount.
